DETAILS OF MAINTENANCE CONTRACT : 2 visits per year: 1 in the spring & 1 in the fall Priority Service 10% off parts

Only the items specified in this contract and/or the attached addenda will be performed. ACCEPT NO VERBAL ASSURANCES FROM ANY REPRESENTATIVE. Any changes or deviation from the specifications herein described will be executed only upon written order of same (“change order”) and will become an extra charge over and above the sum mentioned in this contract. Job cost DOES NOT include fees for permit(s) required by any municipality having jurisdiction over Buyer’s property.

PAYMENT TERMS $199 --- A/C & Heating Maintenance Plan $109 --- Individual A/C Maintenance Plan or Individual Heating Maintenance Plan $249 --- Commercial Roof Top Maintenance Plan
